As a reminder, services requiring prior authorization are screened against medical necessity criteria, Medicare guidelines and/or UnitedHealthcare policy. Procedures that do not require initial medical necessity review must still meet medical necessity criteria, Medicare guidelines and/or UnitedHealthcare policy, and are subject to retrospective review.
