Key Peoples Health Initiatives for 2025

We’ve launched initiatives for 2025, focusing on specific care aspects to facilitate optimal health outcomes for your Peoples Health patients. These include:
- Completing common lab work to monitor health and reduce risk of certain conditions
- Reducing hospital readmissions through post-discharge medication reconciliation (PDMR) during a follow-up visit or phone call within seven days of a patient’s discharge
- Completing a Medicare annual wellness visit once per year per patient
